• CERTIFICATION OF CORRECTION OF VIOLATION(S) (in PDF)  This form is for use when all violations on a Notice of Violation cannot be certified at one time.  This form must be completed properly, including identifying the original NOV ID, signing the form and having the form notarized, for it to be accepted by HPD.  The form must be returned to the same borough office indicated on the original Notice of Violation and will only be accepted for timely certifications (certifications submitted by the certification date identified on the original Notice of Violation.)

Lead-Based Paint Treatment:

• Annual Notice toTenant or Occupant in Buildings with 3 or More Apartments: Protect YourChild from Window Falls and Lead Poisoning available in English (in RTF) and Spanish
  Between January 1st and January 15th annually (or with the January rent bill if issued before January 1st), owners must send a lead notice to all tenants in pre-1960 multiple dwellings and in multiple dwellings built between 1960 and 1978 if owners know there is lead paint in the building. Since owners must also send out window guard notices, this is the only joint window guard/lead approved form.

• Department of Health and Mental Hygiene (DOHMH) Work Notification Form (in PDF): Building owners must use this form to give 10 days advance notice to the DOHMH prior to undertaking work that disturbs more than 100 square feet or replaces two or more windows in an apartment with a child under 6, where the building was either (a) built before 1960 or (b) built between 1960 and 1978 and the owner knows there is lead paint.

Online Property Registration Form: HPD is pleased to introduce our new online property registration form for residential owners. By completing your registration form online, you will instantly be alerted of any errors or information missed. You are still required to print the form and mail the signed copy with payment to HPD. Affidavit in Lieu of Registration:  The attached form is used when applying for a deed with NYC's Department of Finance if the property classification does not require that a property registration be filed with HPD.  This form is submitted directly to the Department of Finance.
